Make sure you read this carefully before signing. No application for renewal of membership, license or endorsement will be considered or processed without this application being completed in full, signed, and dated. All renewal packages may take up to 3 to 4 weeks for delivery once received at the home office.

Please Note: Any member who make a firm commitment of financial support of at least $25.00 a month for the stability and advancement of Chaplain Fellowship Ministries will have their membership automatically renewed each year.

(1) Master Chaplains who did not keep their financial commitment last year or let their membership expire for more than 120 days must contact the home office to get membership renewal approvaled. All records are checked and verified. Renewal packages may take up to 3 to 4 weeks for delivery once received in our office. If you have any questions you can contact the home office and speak with a senior staff member.

(2) This year I am making a monthly financial commitment of at least ____$20.00 ___$25.00 ___$45.00 ___$65.00 ___$75.00 ___$100.00 ___$200.00 ___$300.00 ___$400.00 ___$500.00 for the stability and advancement of Chaplain Fellowship Ministries. Note: The leadership and members of Chaplain Fellowship Ministries have a zero tolerance policy regarding senior chaplains who do not keep their committment without just cause, communication is everything.

I understand that Master Chaplains who fail to keep their monthly commitment will be removed from the membership rolls and will not be allowed to renew their memberships without approval from the home office.

Chaplain Fellowship Ministries Holds all Members Accountable For Their Action - Without Accountability There Is No Credibility

Unscriptural conduct, failure to maintain membership requirements in good standing, or doctrinal departure from the tenets of faith, shall be considered sufficient grounds upon which any member may be disqualified and removed as a member of Chaplain Fellowship Ministries. Such discipline shall be prayerfully administered according to Scriptures by the governing board of directors (Matt. 18:15-17; Rom. 16:17; 1 Cor. 5:9-13; 2 Thess. 3:6). Members agree to the following: If any member is ever charged with a criminal act/sexual misconduct or assault, ethical misconduct "Conduct unbecoming of a chaplain or minister" that this document authorizes their employer or volunteer chaplaincy organization to release all information to Chaplain Fellowship Ministries or their designated investigating agency.

I hereby certify that the above information is true to the best of my knowledge. I understand that Chaplain Fellowship Ministries or affiliate ministries will be relying upon this information to grant me approval regarding ordination, license and endorsement. I acknowledge that any significant and material misstatements made by me will be just cause to deny membership, license, ordination or endorsement. Should any of my responses to these statements change, I will immediately notify the Governing Board of Directors of Chaplain Fellowship ministries.

Mail your completed application to the address listed below after making your donation. Please Note: There will be four dollars added to the cost of the package for PayPal charges. Renewal membership packages may take up to 2 to 3 weeks for delivery once received in our office.

Please Note: Unscriptural conduct or failure to maintain membership requirements in good standing or doctrinal departure from the tenets of faith shall be considered sufficient Grounds upon which any person may be disqualified and removed from membership. The requirements and information required within this application shall be subject to change without notice. The board of directors does hereby have the right to refuse membership renewal for Unscriptural conduct or failure to maintain membership requirements in good standing. All decisions made by the board of directors are final and binding. No goods or services were provided for donations other than intangible religious benefits.
